Clinical characteristics of the state of the autonomic nervous system and their correlations with psychometric indicators in patients with duodenal ulcer in the stage of exacerbation and remission

Aim: to study the clinical characteristics of the state of the autonomic nervous system and their correlations with psychometric indicators in patients with duodenal ulcer in the stage of exacerbation and remission.

Object and research methods. A comprehensive examination of 84 patients with duodenal ulcer in the exacerbation stage and again during remission was carried out. The age of the patients was 25–60 years, the average age was 39.90±1.29 years. Patients were divided into two groups according to the presence of exacerbation or remission of the disease. The control group consisted of 30 practically healthy persons without somatic pathology. The comprehensive examination included a survey with an analysis of complaints, a clinical and neurological examination, a study of the vegetative status with the help of O.M. Vein questionnaire, neuropsychological testing, in particular, determination of the level of personal and reactive anxiety according to the Spielberger — Khanin scale, depression according to the Beck scale, assessment of well-being, activity and mood questionnaire.

Results. When evaluating the subjective scale of the O.M. Vein questionnaire, filled by the patient himself, autonomic dysfunction was found in 76 (90.47%) patients in the exacerbation stage and in 51 (60.7%) in the remission stage of the disease. Objective symptoms of autonomic dysfunction assessed by a physician were found in 70 (83.3%) and 45 (53.5%) patients, respectively. A direct correlation between the number of points obtained according to the objective scale of the O.M. Vein questionnaire and the level of personal (r=0.61) and reactive anxiety (r=0.59) was found. In patients with duodenal ulcer in the exacerbation stage, a correlation was noted between the indicators of the objective scale of the O.M. Vein questionnaire and the level of depression (r=0.53). An inverse correlation (p<0.05) of the indicators according to the well-being, activity and mood questionnaire with the indicators of the O.M. Vein questionnaire (both objective and subjective scales) was found.

Conclusion. In patients with duodenal ulcer, there are manifestations of autonomic dysfunction, both in the stage of exacerbation and remission of the disease. Comparison of test results in dynamics (in exacerbation of duodenal ulcer and in remission) made it possible to conclude that in remission the manifestations of autonomic dysfunction significantly decrease (p<0.05).
