Modern clinical and pharmacological aspects of organic nitrates use in stable coronary heart disease patients.

Despite the wide introduction of modern methods of myocardial revascularization and use of highly efficient drugs with the ability to reduce the risk of cardiovascular complications and mortality, more than a third of patients with stable coronary heart disease (in Ukraine — more than 80%) requires the antianginal treatment. In the article current approaches of the organic nitrates use are summarized — anti-ischemic drugs can affect by the two main pathogenic mechanisms of coronary heart disease: they decrease myocardial oxygen demand by reducing pre- and afterload and increase oxygen delivery to myocardial tissue, especially in its ischemic areas. The choice of nitrates based on their pharmacological properties, clinical feasibility and details of current clinical guidelines.
